If you require any vaccinations relating to foreign travel you will need to complete a Pre-travel Questionnaire which you can obtain from reception. Please fill this in with details of your trip. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required. Once completed, please hand the form to reception and we will arrange a time for the nurse to check your form, as well as book you an appointment to receive the vaccinations.

It is important we receive your completed Pre-travel Questionnaire as soon as possible, as we will then make you an appointment to receive the vaccinations. These vaccines have to be ordered as they are not a stock vaccine. Your appointment needs to be at least 6 weeks before you travel to allow the vaccines to work and for some vaccines you may need a course.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.

Travel Vaccine Shortage

Due to global manufacturing problems there is currently a severe shortage of Hepatitis A and Hepatitis B vaccines. As a result we may not be able to offer you all of the vaccines we would normally recommend. Our Practice Nurses can advise you further but please complete a Pre-Travel Questionnaire in advance of your appointment.
